New incidents are saved as Submitted Status and routed to the Club Welfare Officers for Review.
Incident details
What happened, who was involved, where.
Concise headline (what happened, who, where). Example: “Head collision during U14s training at Arrow Vale — suspected concussion.”
Include: timeline, location specifics, surface/conditions, who was involved, immediate effects, witnesses, actions already taken, and any safeguarding concerns.

Leave Primary player blank for spectators/umpires/volunteers.
Name of the person involved if you can’t pick from the list.
Coach supervising the session.

Injury

Choose the primary injury; if unsure pick Other and add detail in Description.

  • Suspected concussion, safeguarding concerns, or hospital treatment must be logged in EH SportSmart.
  • Reviewers will record when it’s submitted to EH.
Include enough detail for reviewers to act.
