Injury/Incident Reporting Policy

Our responsibility to record and report injuries in line with England Hockey requirements

England Hockey requires all affiliated clubs to record and report certain injuries.
This policy ensures Redditch Hockey Club complies fully, and that members and officials know their responsibilities when injuries occur.

All injuries must be reported

  • Applies to training sessions, matches, social events and on club premises
  • Minor and major injuries must both be logged
  • Primary method: Club Portal (Incident Report form)
  • Backup method: Manual Injury Reporting Form (available on Spond and website)

Why & Who

Why we report:

  • EH monitors injury trends nationally
  • Data informs safety improvements and FIH rule changes
  • Helps clubs learn and improve welfare practices

Who reports:

  • Team Captains / Managers complete the EH online form
  • Welfare Officer keeps local records
  • All members should notify officials if treatment is needed

Special considerations

Young People U18:

  • Inform parent/guardian immediately
  • Follow up with courtesy check-in within 24 hours

Injury severity:

  • Minor: log in Portal/manual form with details
  • Major: fractures, concussion, loss of consciousness, severe bleeding, hospital treatment → seek medical help immediately & submit report within 24h

Key areas

  • Reports assigned as tasks to Welfare Team
  • Officer reviews, contacts involved, manages case, records findings, closes task
  • Reviewed by Committee at meetings

All injuries recorded with details: date, time, location, person, witnesses, treatment, follow-up

  • First aider required at every session
  • Kits at pitch-side, hockey garage, clubhouse bar (incl. bleed kit), defibrillator on exterior wall
  • Captains/coaches responsible for team kit stock
  • Kit Co-ordinator supplies replacements
  • Members must keep Portal details (medical/emergency contacts) up to date

  • Follow EH guidance
  • Remove immediately if suspected
  • Must complete GRTP (Graduated Return to Play) before resuming

Information shared only with those who need to know (first aiders, medical staff, Welfare, Committee where necessary)

  • Exec Committee reviews injury trends
  • Only anonymous data shared at Committee
